Nurse Navigator Oncology Clinic
About The Position
A professional registered nurse who provides individualized assistance to patients, families, and caregivers to help overcome health care system barriers. The Nurse Navigator enhances the patient experience and clinical outcomes by providing seamless navigation throughout the continuum of care. The role collaborates with physicians and interdisciplinary team members to triage, coordinate, and manage patient care; educates patients; and serves as a point of contact for patients and families. The position participates in strategic initiatives within the clinical program or service line, including developing, implementing, and monitoring navigation activities and their sustainability. It facilitates shared decision-making and patient engagement by building trusting relationships, addressing communication and health literacy needs, and helping patients incorporate their values and preferences into treatment decisions. The role promotes positive outcomes for patients, providers, and the Sutter health care system.
Requirements
- Graduate of an accredited school of nursing.
- California Registered Nurse (RN) licensure.
- Basic Life Support (BLS) Healthcare Provider certification.
- Five years of recent relevant experience.
- Knowledge of medical terminology.
- Ability to follow through on responsibilities and communicate with varied levels of health care staff.
- Verbal and written communication skills.
- Experience with Microsoft Office software, including Excel and Word.
