About The Position

The GW Medical Faculty Associates is looking for a qualified Nurse Navigator to provide proficient nursing care for an individual or group of patients with less predictable problems and to provide leadership in the implementation of high-quality nursing care. The position will provide support to the departments of both Renal Medicine and Pulmonary Medicine.

Requirements

  • An Associate's degree in Nursing or an equivalent combination of training and experience.
  • Current D.C. Licensure as a Registered Nurse or evidence of application for licensure in D.C. and current out-of-state is necessary.
  • Sit, walk, reach, bend, or twist for long periods of time in a clinical setting.
  • Must be able to lift, carry, push, or pull up to 100 lbs. as part of the role.
  • Regularly exposed to healthcare settings that may require personal protective equipment.
  • Requires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment.

Responsibilities

  • Uses advanced nursing knowledge and the nursing process to assess factors of the patient’s past and present physical, psychosocial, and cultural baseline, health problems, and external support systems that may impact his/her ability to cope with the current health situation.
  • Defines a nursing problem list based on the assessment of the patient, based on the consideration of the medical diagnosis and the patient’s problems.
  • Defines realistic, measurable goals related to each problem.
  • Develops a nursing plan that is directed toward the optimal resolution of nursing diagnoses and prevention of future problems.
  • Evaluates and documents a patient’s progress toward goals.
  • Collaborates with the appropriate healthcare professionals in implementing a comprehensive plan of care.
  • Provides an individualized plan of nursing care that assists the patient in maximizing participation in his/her care.
  • Collaborates with a physician to revise the nursing plan of care and/ or goals as necessary.
  • Implements appropriate patient education programs to meet individual healthcare needs.
  • Assists physicians with day-to-day patient care, including taking histories and assessing the reason for visits.
  • Assists with procedures and treatments.
  • Provides patient teaching when necessary.
  • Records nursing care using appropriate documentation.
