Garrison Women's Health (GWH), located at 770 Central Avenue in Dover, NH, consists of a premier team of expert providers and specialists who take a thoughtful approach to provide high-quality compassionate care in the areas of Obstetrics, Gynecology, Infertility, and LGBTQ+ care. For more than 50 years, GWH's experienced physicians, midwives, and ancillary staff have cared for women in the greater New Hampshire Seacoast area. We are seeking a full-time, 40-hour High-Risk Nurse Navigator with a schedule of Monday–Friday, between the hours of 8:00 AM–5:00 PM, with two set remote workdays each week. This role is based at 770 Central Ave (Garrison Women's Health) and will support a dynamic clinical team. This role may include triage responsibilities, nurse visits, and telephone-based patient care. The High-Risk Nurse Navigator will primarily focus on managing a panel of approximately 150 high-risk patients, providing comprehensive care coordination and ensuring continuity across services. This position plays a key role in delivering patient-centered care and collaborating closely with internal teams. Job Summary Responsible for coordinating the complex care needs of patients and is the primary patient contact for service access, diagnostics, treatment activities, schedules, patient and family education, and multidisciplinary collaboration.
