About The Position

The Navigator functions as a coordinator for patients through an episode of care. The Navigator collaborates with surgeons, primary care providers, rehabilitation specialist, and other health team members to provide high-quality care for surgical patients following established standards and practices. The delivery of professional nursing care at Stormont-Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared Governance, both of which are congruent with the mission, vision, and values of the organization.

Requirements

  • Bachelor's of Science in Nursing (BSN)
  • 3 years Professional Nursing experience.
  • Registered Nurse - KSBN Required
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to read, analyzes, and interprets general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to effectively present information and respond to questions from physicians, patients, co-workers and the general public.
  • Professional nursing theory and practice to give and evaluate patient care: • Medical equipment and instruments to administer patient care • Common safety hazards and precautions to establish a safe work environment
  • Ability to define problems collects data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of technical instructions in the mathematical diagram form and deal with several abstracts and concrete variables.
  • Applying and modifying the principles, methods and techniques of professional nursing to provide on-going patient care.
  • Developing and maintaining department quality assurances.
  • Establishing and maintaining effective working relationships with patients, medical staff and the public.

Responsibilities

  • Initiates communication with patients to discuss procedures, processes, and resources.
  • Efficiently documents medical information using the appropriate forms and/or electronic applications.
  • Utilizes EPIC to gather data as appropriate documenting all patient interactions.
  • Maintains and reviews patient records.
  • Tracks interventions and outcomes.
  • Guides patients through the health care system.
  • Connects patients to community and social support services.
  • Assists patient in selecting educational programs.
  • Evaluation of patients to determine that program specific criteria is met.
  • Triages and refers incoming telephone calls.
  • Oversees appointment scheduling.
  • Maintains timely flow of patients.
  • Verify benefits and obtain authorizations.
  • Contact insurance companies to conduct and complete benefit investigations.
  • Conducting initial patient intakes (collecting patient information, insurance information, referral source, brief relevant clinical information, etc.).
  • Delegates tasks and duties in the direction and coordination of health care team members, patient care and department activities in accordance with the Kansas State Nurse Practice Act.
  • Self-directed and purposeful in seeking the necessary knowledge and skills to enhance professionalism in order to provide high-quality care.
  • Participates in system wide/department patient care quality improvement activities as well as standards development.
  • Demonstrates knowledge of the Kansas State Nurse Practice Act and follows the statutes and regulations therein.
  • Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health.
  • Performs other duties as assigned.
