The Nurse Navigator (RN) - Oncology/Breast Surgery position is a full-time role responsible for the coordination and implementation of strategies to ensure quality, efficacy, and cost-effectiveness for Breast Surgery/Oncology patients. This role involves assessing patient needs, orienting patients to the hospital and outpatient settings, and delivering cancer care across the continuum from diagnosis through treatment, transition to survivorship, and end-of-life care. The Nurse Navigator provides patient and family education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care. They critically think and clinically assess patients, manage the complexity of cancer diagnoses, and collaborate with multidisciplinary team members. The role also includes reviewing distress screenings for referrals, reinforcing education regarding diagnosis, connecting patients to appropriate resources, and developing/reporting outcome metrics for program development. Additionally, the Nurse Navigator provides emotional support, advocates for the patient, empowers self-advocacy, and maintains their own knowledge, abilities, skills, and competencies while role-modeling organizational behaviors of excellence.
