About The Position

Under general administrative direction, provides a significant level of patient education related to illness and planned management and treatment and facilitates coordination of care for INFECTIOUS DISEASE patients. In addition, the Nurse Navigator provides in-clinic support to providers, as needed.

Requirements

  • An Bachelor’s degree in Nursing or higher, or an equivalent combination of training and experience.
  • Current D.C. Licensure as a Registered Nurse or evidence of application for licensure in D.C.
  • A minimum of 2 years of experience as a Registered Nurse is required.
  • Sit, walk, reach, bend, or twist for long periods of time in an office setting.
  • Must be able to lift, carry, push, or pull up to 20 lbs. as part of the role.
  • Regularly exposed to healthcare settings that may require personal protective equipment.
  • Requires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment.

Responsibilities

  • Liaison for ID patients requiring coordination of care and support.
  • Work collaboratively with the administrative and clinical ID teams to manage clinical issues; urgent tasks, prescription pre-authorizations and refills, appointments.
  • Coordinate and manage all urgent orders for dynamic testing and procedure referrals to include patient preparation and education.
  • Follow up with patients on testing and procedure results in coordination with providers.
  • Provide appropriate clinical support, counseling, and education. Alerts provider to urgent situations that cannot wait until scheduled follow-up.
  • Prepares Clinic prep notes for new patients and returning patients which includes scanning notes/imaging, contacting referring physicians or imaging groups, and contacting patients on any issues on items needed for visit.
  • Prepares records needed and reconciled for future appointments.
  • Assist providers in addressing insufficiencies in care provision by coordinating follow-up, orders, and referrals.
  • Participates in quality improvement, patient safety, and patient experience initiatives.
  • Measures and tracks departmental progress in meeting quality, safety, and patient experience goals.
  • Participates in training and education of support staff.
  • Keep up-to-date records/data for programs and patients within MFA and GWUH.
  • Identify and coordinate health maintenance issues.
