Requirements

  • Bachelor’s degree in nursing, or a related field
  • 0-2 years of experience in nursing, or a relevant area
  • An equivalent combination of education and experience may be considered.
  • All experience must be paid and in the same related field.
  • Part-time and PRN experience will be prorated based on hours worked per week.
  • Volunteer work and internships for academic credit are not counted.
  • Registered Nurse

  • Master’s degree in nursing is preferred

Responsibilities

  • Provides advocacy, education, and emotional support to empower informed decision-making and enhance the patient and family experience.
  • Collaborates with multidisciplinary teams to ensure seamless transitions, address barriers to care, and optimize health outcomes.
  • Ensures effective communication among patients, providers, and community resources.
  • Promotes continuous improvement in care delivery through patient feedback, program development, and ongoing professional education.
  • Provides individualized support, education, and information to patients and families throughout the care continuum, including disease process, treatment, and community resources.
  • Serves as the primary liaison and communication link between patients, families, medical staff, and the care team to ensure seamless, coordinated care.
  • Assesses, plans, implements, and evaluates patient care and education needs, documenting interventions and outcomes.
  • Identifies and addresses barriers to care, assisting with logistical, financial, and psychosocial needs, and referring to supportive services as appropriate.
  • Coordinates and monitors patient progress, compliance, and transitions of care, providing ongoing feedback and support.
  • Designs, develops, and participates in the creation and delivery of health education materials and programming for patients and staff.
  • Maintains advanced specialty training, certifications, licensure, and completes all required competencies and continuing education.
  • Collaborates with interdisciplinary and community partners to facilitate multidisciplinary care and promote evidence-based practice.
  • Collects, analyzes, and reports clinical data to support quality improvement, safety, and program goals.
  • Performs other duties as assigned.
