About The Position

Patient Care Nurse Navigator for Digestive Health at Advocate Condell Medical Center in Libertyville, IL. This is a Full Time position with 40 hours per week, scheduled Monday-Friday from 8am-4pm. The role involves no holidays, weekends, or on-call duties. The Nurse Navigator will coordinate patient care to improve clinical outcomes, manage patient transitions, and implement evidence-based disease management protocols. This position requires a BSN and at least 3 years of professional RN experience in the care of endoscopy and GI surgical patients. Ambulatory clinical experience is preferred. The role demands a global perspective of the changing healthcare environment, strong prioritization and computer skills, and effective interpersonal communication. Current Illinois RN license and CPR certification are required. Additional certifications such as MedSurg, CGRN, CAPA, or Nurse Navigator Certification are expected within two years of hire. The position involves potential exposure to hazardous materials and life-threatening diseases, and requires the ability to perform patient lifting/transfer activities (up to 50 lbs may be required) and travel to visit physicians and other practices within the region. The role also requires recognizing the needs and behaviors of specific age groups of patients treated.

Requirements

Responsibilities

  • Demonstrates effectiveness in promoting professional practice and resource management that will optimize efficiency and safety through unit operations.
  • Adherence to and holding others accountable to professional guidelines for practice, professional standards, and ethical practice guidelines.

  • Provides feedback in review and revision of policy, procedure, and clinical practice guidelines/standards.
  • Promotes interdepartmental collaboration and communication in order to provide seamless, safe, patient centered care.
  • Develops a partnership and effective communication with the multi-disciplinary care team/ departments to coordinate safe, timely and efficient patient care strategies.
  • Coordinates and triages post-discharge phone calls.
  • Proactively identifies appropriate patients and utilizes the appropriate screening tools to identify risk factors.
  • Coordinates with Care management to begin the discharge plan and goals of the DASC and STARR patient as appropriate.
  • Oversees the DASC and STAAR patients by functioning as a liaison to the patient care team. This includes: rounding with staff following the patient to enhance communication, attend unit rounds as needed, coordinating the care and information with other providers facilitating resolution of issues that could affect patient care.
  • Assesses patient needs, communicates the plan, evaluates the effectiveness and revises the plan as needed to reflect changing needs, outcomes, age and risk factors.
  • Collaborates with PAT to partner with physician offices participating in DASC and STARR. Maintains effective and up-to-date communication for the interdisciplinary team. Serves as a resource to PAT in relaying pertinent patient care information.
  • Assesses needs, communicates the plan, evaluates effectiveness, and revises the plan to reflect changing needs, outcomes, age and developmental status. Manages the patient transition through the process.
  • Represents Perioperative Services on specific hospital teams.
  • Acts as a resource to nursing staff for DASC and STAAR patients.
  • Develops and provides education to DASC and STAAR patients and families related to procedure, disease process, care requirements and community resources.
  • Maintains statistics related to compliance with protocols/guidelines of STAAR patients through the surgical episode of care.
  • Maintains statistics related to compliance results for DASC patients through the endoscopy episode of care.
  • Empowers the patient to self-advocate.
  • Maintains own knowledge, ability, skills, competencies.
  • Continues to role model organizational behaviors of excellence.
