The Nurse Navigator (NN) serves as a multidisciplinary team leader and liaison among patients, family, physicians, clinical care teams, community organizations, and BHSF leadership. This role involves leading the coordination of care across the continuum, providing education, support, and resources to patients and their families, facilitating shared decision-making, advocating for patients, ensuring timely access to quality multidisciplinary care, and promoting patient satisfaction. The NN is also responsible for monitoring individuals, populations, and program outcomes and engaging in quality improvement activities.

Requirements

  • Bachelor Science Nursing (BSN) degree
  • Active Registered Nurse license
  • Certification in area of expertise
  • Advanced Cardiovascular Life Support (ACLS)
  • Basic Life Support (BLS)
  • Neonatal Resuscitation Program (NRP)
  • Pediatric Advanced Life Support (PALS)
  • 5 years nursing experience in key relevant areas such as MedSurg, Oncology Care (Inpatient/Outpatient Oncology, Infusion, Radiation, BMT, etc.), Patient Navigation or related service (Case Management, Care Coordination, Social Work, Population Health, etc.), or specific Disease Sites (Breast, GI, GU, Neuro, etc.)
  • Skilled in Microsoft Office applications (Word, Excel, Sharepoint, TEAMS)
  • Skilled in EMR applications (HEDIS, STAR, etc.)
  • Typing skills
  • Life Support Certificate (BLS, NRP, PALS, ACLS) as per department standard

Nice To Haves

  • Experience working in a hospital, managed care or medical office
  • Bilingual English/Spanish
