About The Position

Under the supervision of the Old Colony Y Senior Executive Director, the MAP Nurse Monitor is responsible for ensuring that all residents in the program receive quality medical intervention as ordered by assigned providers. The MAP Nurse Monitor is responsible to carry out all MAP policies in a manner consistent with Old Colony Y and all State Regulatory agencies.

  • Certification as a MAP approved trainer
  • Ability to work within three distinct programs
  • Previous professional experience with working with adolescents

Responsibilities

  • Provide quality assurance by monitoring our MAP site’s medication administration system to ensure compliance with DPH, DMH, and DCF regulations, licensing requirements and Map policies.
  • Assess and observe, no less than once every 12 months, all certified program MAP staff to verify that they are following correct MAP policies, procedures and protocols in preparing and administering medication in a manner consistent with all MAP policies, procedures and protocols. The observation shall be documented on an agency form (that includes staff name, certification number, date of observation, and nurse monitor signature) and kept on file at the program site.
  • Verify appropriate documentation of all Certified staff administering medication at the site.
  • Report deficient practice by Certified staff in accordance with MAP Policies.
  • Work collaboratively with program supervisory staff to identify and address, individually and collectively, medication administration related remedial training needs for Certified staff.
  • Verify that the prescribing practitioner’s orders are on site, current, and properly transcribed.
  • Verify that medications dispensed by the pharmacy correspond with those medications prescribed by the practitioner.
  • Ensure that verbal orders from prescribers are accurately transcribed and implemented.
  • Ensure that medications are arriving as ordered from the pharmacy and are being stored and administered in accordance with all applicable statute, regulation and policy.
  • Ensure that Court-ordered medication(s) subject to a “Rogers” order are administered as directed by the Court.
  • Review medication logs at regularly scheduled intervals to ensure that medications are being administered in a manner consistent with the prescribing practitioner’s orders.
  • Consult with and educate staff regarding youth with eating disorders and other medical conditions, e.g., diabetes.
  • Provide medication education for youth and staff and ensure families receive education regarding side effects of medication.
  • Educate staff and youth regarding infection control issues, including influenza transmission.
  • Work closely with youth with complex medical needs and participate, as clinically indicated, in treatment planning.
  • Be available for consultation for medical issues and to oversee the establishment, implementation and review of medical protocols as related to youth’s medication orders.
  • Assess issues and the technical functions related to the medication administration for our various populations that may be unique to individuals served at a particular site e.g., side effects, liquid medications, vital signs, blood glucose monitoring, allergies and takes steps to ensure that Certified staff are properly informed, educated and/or competent with regard to such issues.
  • Assure that current versions of the MAP Policy Manual and the MAP Curriculum are readily available at all programs.
  • Review Medication Occurrence Reports (MORs) to determine areas where further training or other intervention may be needed and ensure timely submission and follow up of Medication Occurrence Reports.
  • Review documentation of medications being administered at the site to ensure that proper clinical protocols are in place and are being observed, e.g., monitoring vital signs, blood glucose monitoring, and appropriate practitioner notification.
  • Consult with healthcare providers regarding youth with complex medical needs, and, coordinate with outside medical providers.
  • Communicate with state MAP Coordinators regarding findings relative to the Technical Assistance Reviews performed at our sites.
