About The Position

The Nurse Manager (DON) at Brookstone Estates of Effingham is responsible for the comprehensive health management of residents. This includes conducting initial and wellness assessments, developing individualized care plans, and providing health promotion, disease prevention, and self-care education. The role requires compassionate engagement with residents and their families, as well as fostering communication with physicians and other healthcare providers. A key leadership component involves hiring, training, scheduling, and leading the clinical team to ensure exceptional care. The Nurse Manager also coordinates the community's infection control program, maintains state regulatory compliance, develops and oversees the medication management program, and contributes to quality assurance and performance improvement initiatives, all while adhering to confidentiality and privacy regulations.

Requirements

  • Maintains current state nursing licensure. LPN required
  • Minimum of two (2) years nursing supervisory experience
  • Excellent written and verbal communication skills
  • Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members
  • Demonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignity

Nice To Haves

  • RN preferred
  • Prior senior/supportive living experience strongly preferred

Responsibilities

  • Conducts initial assessment of prospective residents, including off-site visits
  • Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident
  • Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents
  • Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
  • Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members
  • Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences
  • Coordinates the Community’s infection control program while maintaining state regulatory compliance
  • Develop and oversee the Community’s medication management program
  • Contributes toward quality assurance and performance improvement initiatives
  • Complies with all rules and regulations regarding confidentiality and privacy of resident information
