About The Position

Eliot is seeking a licensed, experienced nurse to join its Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model providing comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness. The position provides nursing care, case management, outreach, and support, including in individuals’ homes. The ideal candidate has knowledge of psychiatric medications and psychiatric and medical treatment.

Requirements

• Associate degree in nursing.
• LPN or RN nursing license.
• Experience working with adults with mental illness.
• Knowledge of psychiatric and medical medications.
• Valid Massachusetts driver’s license.

Responsibilities

• Act as a primary case manager to 3–6 consumers.
• Provide nursing interventions to individuals.
• Facilitate and coordinate medical care, including communication with health care providers.
• Provide health education to individuals.
• Assist with medication management and the overall functioning of the nursing department.
• Formulate and regularly update Comprehensive Assessments and IAPs for clients on the caseload.
• Transport consumers to work sites, meetings, daily activities, appointments, and other activities as needed.
• Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients.
