About The Position

Under the direct supervision of the Medical Director and general oversight of the Director of Nursing, the Nurse Manager/Nurse Practitioner serves in a dual clinical and leadership role, ensuring the delivery of high-quality medical and psychiatric care across assigned clinic sites. In the Nurse Practitioner capacity, this position provides comprehensive psychiatric care to patients, including conducting mental health assessments, managing medications, and developing individualized treatment plans. Responsibilities include obtaining health histories, performing physical assessments, identifying clinical concerns, and delivering treatment, health education, and counseling. The Nurse Practitioner collaborates closely with an interdisciplinary care team to support continuity and quality of care. In the Nurse Manager capacity, this role oversees the medical aspects of program services within a designated region and provides supervision, guidance, and clinical oversight to Registered Nurses (RNs) and Licensed Practical Nurses (LPNs). The Nurse Manager ensures adherence to clinical standards, supports staff development, and promotes a culture of excellence in patient care. A strong commitment to RHD values is essential, with an expectation to model these principles through both clinical practice and leadership. This is role will provide support between several RHD programs and will be a back up prescriber to current psychiatrists and medical director.

Requirements

  • Current licensure as a Registered Nurse (RN) and certification as a Family Nurse Practitioner (FNP), including furnishing number.
  • Active DEA license required.
  • Current CPR certification required
  • Minimum of one (1) year of experience working in a community clinic setting.
  • Three (3) to five (5) years of experience working with individuals with intellectual and developmental disabilities (IDD), co-occurring behavioral health and substance use disorders, homelessness, community support services, group residential settings, or similar populations.
  • One (1) to two (2) years of supervisory experience, preferably in a clinical or community-based healthcare setting.
  • Demonstrated experience in the delivery of psychiatric care.
  • Strong understanding of and sensitivity to diverse patient populations.
  • Excellent verbal and written communication skills.
  • Ability to work both independently and collaboratively as part of a multidisciplinary team.
  • Strong clinical judgment with the ability to assess patient needs, analyze complex situations, and make sound decisions under pressure.
  • Detail-oriented with the ability to effectively prioritize and manage multiple tasks.
  • FBI clearance (as required) and completion of all trainings required by licensure.

Responsibilities

  • Provide comprehensive psychiatric care to members, including obtaining health histories, conducting physical and mental health assessments, ordering and reviewing lab tests, prescribing medications, and making appropriate referrals in accordance with current clinical guidelines and protocols. This does include Detox Admissions and will be a back up prescriber, when needed.
  • Develop and implement individualized treatment plans, including medication management, health education, counseling, and follow-up care.
  • Accurately and timely document all patient interactions, including histories, subjective findings, examinations, treatment plans, prescriptions, and follow-up, within the Electronic Health Records (EHR) system in compliance with RHD policies and Medicaid/Medicare guidelines.
  • Ensure patient chart entries follow proper progress note format and regulatory standards.
  • Provide patients with clear information regarding their evaluation, diagnosis, treatment options, and prognosis, actively engaging them in their healthcare decisions.
  • Deliver counseling and additional support to patients as needed.
  • Coordinate care effectively with internal teams, external providers, and hospitals to ensure continuity and high-quality outcomes.
  • Consult with supervising physicians or colleagues on complex cases or when outside scope of practice.
  • Participate actively in quality improvement initiatives to enhance clinic efficiency and patient care outcomes.
  • Supervise Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) within the assigned program or region.
  • Conduct regular supervision meetings, performance evaluations, and provide mentorship, training, and professional development support for nursing staff.
  • Partner with Program Directors to recruit, interview, and onboard nursing staff to support program needs.
  • Promote a collaborative, respectful, and accountable work environment aligned with RHD values.
  • Manage nursing schedules along side our Nursing Administration Coordinator, including coverage for callouts, no-shows, and time-off requests to ensure continuous patient care.
  • Develop, implement, and maintain nursing policies, procedures, and clinical protocols to ensure safety and regulatory compliance.
  • Oversee and improve clinical documentation tools, including assessment forms and nursing notes, to support communication and quality care.
  • Perform regular chart audits to ensure accurate and compliant documentation reflecting patient medical and behavioral health status.
  • Provide required federal and state ambulatory care data and reporting as applicable.
  • Notify Facility Administration of any issues that may impact the efficiency or effectiveness of the medical department.
  • Implement physician orders and ensure appropriate clinical follow-through.
  • Oversee the medical and psychiatric care needs of individuals served, facilitating communication among members of the interdisciplinary treatment team.
  • Assess, monitor, and respond to new, ongoing, or emerging health concerns of patients.
  • Ensure all duties and responsibilities are carried out with the highest regard for the safety and well-being of patients, staff, and self.
  • Address and take corrective action on employee health and safety concerns as appropriate.
  • Demonstrate professionalism and cooperation with supervisors, colleagues, staff, patients, and community partners at all times.
  • Perform other duties as assigned by Supervisor
