About The Position

HealthRIGHT 360, a nonprofit organization, is dedicated to providing accessible and comprehensive healthcare to vulnerable populations, aiming to overcome systemic barriers and promote health equity. The Integrated Care Center clinic, a federally qualified health center, offers a wide array of services including primary care, behavioral health, addiction treatment, psychiatry, and dental care, serving individuals experiencing homelessness, mental illness, substance use disorders, and chronic mental conditions. The clinic operates on a team-based care model within an integrated setting. HealthRIGHT 360 is seeking a mission-driven service leader with experience in staff management and support, a commitment to quality improvement, and a passion for innovation. The Nurse Manager will serve as a clinical leader, providing essential nursing support within a Patient Centered Medical/Health Home model, enhancing team functioning and efficiency. This role is crucial for complex care management of patients with multiple chronic medical and behavioral health issues. The position also requires upholding and promoting a commitment to addressing racism, and gender and sexual discrimination in clinical practice and within the organization.

Requirements

  • Graduation from an accredited school of nursing as a Bachelor's of Nursing.
  • Registered Nurse License by the California Department of Consumer Affairs (DCA).
  • Possession of a valid California license as a registered nurse.
  • Possession of BCLS/CPR certification.
  • A minimum of 3 years of managerial/supervisory experience.
  • Experience with electronic medical records.
  • Two years of direct patient care experience and one year in ambulatory care and/or a community health center.
  • Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and culture backgrounds and economic status.
  • Two or more years of leadership experience in a community healthcare setting; preferred experience in leading other RNs in a multidisciplinary setting.
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.
  • LGBT Cultural competence and able to work with a diverse population.
  • Commitment to working in an environment offering abortion services and the full scope of pregnancy options counseling.
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
  • Knowledge of co-occurring disorders and trauma informed treatment.

Responsibilities

  • Provide nursing care as part of an integrated care team, within the scope of license and agency credentials, to patients within a Patient Centered Medical/Health Home delivery model according to pre-arranged schedules.
  • Document care promptly and follow charting protocols and applicable CA state licensing requirements.
  • Utilize motivational interviewing skills and encourage patient activation and self-management.
  • Appropriately utilize standing orders.
  • Facilitate team functioning and efficiency by taking responsibility for additional patient assessments or medical assistant functions (e.g., phlebotomy, rooming, vitals, etc.) to ensure the completion of patient sessions in a timely and thorough manner.
  • Assess and provide nursing advice to patients who walk in with medical complaints or who call in with medical concerns seeking advice.
  • Respond to patient requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner.
  • Inform clients of normal and abnormal lab results and follow-up plans, as appropriate for scope or under the guidance of a provider.
  • As a part of the care support team, prioritize and take responsibility for patients with complex medical issues and follow up with all patients regarding recent emergency room visits and inpatient stays.
  • Work collaboratively with mental health staff and other ICC services to facilitate integration between primary care, mental health, and other ancillary services in order to improve patient outcomes.
  • Manage a team of nurses to deliver high-quality results and provide ongoing recognition, feedback, and support to develop nurses’ professional skills.
  • In conjunction with medical director or designee, provide clinical leadership to support staff, including Registered Nurses, Dietitian, Medical Assistants, Patient Care Coordinators, and Front Office Administrative Assistants.
  • Work collaboratively with providers and other staff to develop standardized nursing protocols and standing orders.
  • Participate in training Medical Assistants, nursing students, and volunteers to improve the skills of back-office staff as appropriate.
  • Participate as an active member of the Care Support Team through case-conferencing and other collaborative work.
  • Demonstrate comfort in changing systems, champions change and supports change processes.
  • Demonstrate commitment to the mission of the organization, the healthcare of medically underserved people, harm reduction, client centered care, racial justice, and community-oriented primary care.
  • Serve as an active participant in Quality Improvement activities, possibly act as change champion or lead.
  • Work closely with staff collecting and reporting contractual data as necessary.
  • Meet benchmarks for applicable quality indicators as noted in the annual quality management plan including the maintenance of current productivity standards, cycle times, no-show rates, and landing times.
  • Along with colleagues, assists in continuous quality improvement through on-going problems/needs identification and problem solving and as requested, acts as change champion or lead in improvement activities.
  • Maintains quality assurance logs and leads medical assistants and nursing staff to ensure the quality of medications, supplies, medical equipment, and laboratory testing.
  • Keep up to date with operational and procedural requirements.
  • Adhere to company dress code and maintains professional appearance.
  • Complete mandatory training within deadlines.
  • Supervises students and volunteers as appropriate.
  • Maintain availability for various administrative/clinical duties as requested by supervisor (e.g., case presentations).
  • Provide in-service training to staff as needed.
