About The Position

The Nurse Manager is responsible for the development, performance, and management of clinical patient care for nursing and medical assistants. They will work as part of a triad leadership team to make decisions that best support clinical staff, extended care team members, and providers to ensure patients obtain access to care that is timely, effective, efficient, and safe. The Nurse Manager is responsible for the population health management of an assigned clinic patient panel by performing regular analysis of population health data, improving clinical care workflows to enhance the quality of patient care, and increasing cost-effectiveness. The Nurse Manager will also provide oversight and direction related to staff education, onboarding, and training of new hires with a focus on CommUnityCare's care model.

Requirements

Graduation from an accredited school of nursing
3 years clinical experience preferably in a primary care setting or ambulatory clinic with focus in general nursing care - Required
3 years progressive management/leadership experience working in a healthcare environment - Required
3 years Experience leading teams with clinical quality improvement, clinical workflow development, patient care coordination, preventative health, and nursing triage. - Required
Unrestricted Registered Nurse (RN) License in the State of Texas
Basic Life Support (BLS) ‐ Obtained through approved American Heart Association - Upon Hire
High level of skill at building relationships and providing excellent customer and patient care.
High degree of knowledge and competency in the practice of nursing and documentation requirements.
Requisite skills and ability to perform assigned nursing tasks.
High level of problem solving skills to better serve patients and staff.
Strong attention to detail and accuracy.
Excellent verbal and written communication skills.
Strong application of Microsoft office and electronic health records.
Demonstrated knowledge of Joint Commission standards, HIPAA regulations, Patient Centered Medical Home, and Value Based Care.

Responsibilities

Manages the clinical support team.
Implements and provides oversight of clinical care delivered by nursing, medical assistants, and ancillary care team.
Establishes work assignments priorities for responsibility areas including nursing, Medical Assistants and ancillary staff.
Responsible for management oversight of health center equipment needs, equipped examination rooms, infection control standards, CLIA and Radiology regulations, compliance with Standing Delegation Orders, and emergency events.
Monitors the compliance with standards of care for on-site treatments and procedures, administration of medications, as well as timely and quality clinical workflows.
Works with the practice administrator to ensure the clinical services team supports the overall operational and fiscal activities of the site to include staffing levels, budgets, financial and operating goals including meeting productivity and quality goals.
Approves payroll and time off requests.
Administering performance evaluations, recruiting, selecting, monitoring, and orientation of new hires.
Responsible for coaching and counseling staff.
Participates as an effective member of the site Triad management team to support implementation of continuous improvement activities.
Routinely acts as the site lead in the absence of the Practice Administrator.
Ensures timely assessments, interventions, and measurement for corrective action strategies when needed.
Solves patient issues to ensure expected results.
Assures quality of care philosophies and standards of care.
Maximizes quality outcomes for patients’ services through the most appropriate utilization of systems.
Serves on CUC committees as assigned.
Adheres to state board of nursing and state nurse practice act requirements and to other governing agency regulations.
Establishes a compassionate and respectful environment which values teamwork, integrity and dependability.
Maintains supplies inventory by studying usage reports, identifying trends, anticipating needed supplies, approving requisitions and cost allocations.
Reviews and audits monthly quality metrics by viewing and monitoring data.
Reports and develops strategic plans to improve specific sites metrics.
Develops strong communication methods to cascade information.
Builds strong relationship with the team to ensure critical operational metrics and competencies are performed.
Performs other duties as assigned.
