About The Position

The Nurse Home Visitor provides in-home support and education to first-time mothers, guiding them through pregnancy and the first two years of their child’s life. They focus on improving maternal and child health outcomes, supporting family economic self-sufficiency, and connecting clients to community resources as needed. Through building long-term therapeutic relationships, conducting nursing assessments, and collaborating with clients to set goals, they deliver care in alignment with the Nurse-Family Partnership model. As an integrated health organization Acenda values the use of evidence-based practice or EBP's. The Nurse-Family Partnership Program is a free service that pairs a first-time mom by 28 weeks gestation with a personal registered nurse to ensure the best possible pregnancy outcome, the best health and development of the baby, and to help link mom to community resources.

Requirements

Bachelors degree in Nursing (BSN) required
Active NJ Registered Nurse license (in good standing)
Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record
Professionalism and a commitment to excellence in care

Responsibilities

Travel to clients' homes and provide home visits to first-time mothers providing education and support through pregnancy and up until the child's 2nd birthday.
Work to improve pregnancy outcomes, child health and development outcomes, and the economic self-sufficiency of families.
Build long-term therapeutic relationships with clients and their families.
Provide community resources as needed per nursing assessment.
Adheres to the nursing process and the Nurse-Family Partnership model of home visitation.
Attends and participates in case conferences and team meetings.
Collaborates with client and family to develop short- and long-term goals.
