About The Position

The Family Connects New Jersey (FCNJ) Nurse Home Visitor provides in-home nursing care, education, and support to postpartum individuals, new parents, adoptive, resource, and bereaved families. This role is crucial in fostering positive parenting practices and supporting early childhood development. The Nurse Home Visitor builds trusting relationships with clients, conducts health assessments, delivers evidence-based education, and connects families to community resources. Working within a strength-based, culturally responsive framework, this position supports improved health outcomes and family well-being. The Nurse Home Visitor collaborates with interdisciplinary teams and contributes to program evaluation and continuous improvement.

Requirements

  • Bilingual preferred (Spanish, Haitian Creole, Arabic, Korean, or Portuguese)
  • Excellent verbal, written and interpersonal communication skills
  • Proficient with Microsoft Office Suite
  • Associate’s degree in nursing required.
  • Current NJ RN license and BLS certification required
  • Experience working with diverse populations, communities, and organizations.
  • Demonstrated commitment to valuing diversity and contributing to an inclusive working and learning environment
  • Ability to work collaboratively with various agencies and stakeholders
  • Strong documentation skills

Nice To Haves

  • Bachelor’s degree in nursing preferred
  • Minimum two years of experience providing maternal/child health service in community settings, home visitation, OB-GYN nursing, neonatal and/or pediatric nursing, labor and delivery, adult mental health, or closely related fields preferred

Responsibilities

  • Conduct in-person home visits for families during the postpartum period.
  • Complete and document caregiver and newborn physical assessment and systematic assessment of family strengths, risks, and needs
  • Complete the Family Connects International (FCI) model required training and participate in ongoing training webinars and professional development training
  • Build and maintain a caseload of 30 families per month (6-8 visits/week), providing 1-3 visits per family within the eligible time period
  • Utilize the nursing process to assess, plan, and document families' physical, emotional, social, and environmental needs.
  • Provide anticipatory and supportive guidance to families
  • Assess caregiver and newborn physical health, infant attachment, intimate partner violence, substance use disorders, perinatal mood disorders, and connection to health and community services.
  • Provide referrals to needed and desired community services for families
  • Contribute to and participate in case conferences, quarterly reviews, and other activities to support the delivery of services with quality and fidelity
  • Collaborate with outreach staff regarding community referral sources.
  • Support the program to develop and maintain community and provider referral sources
  • Attend and represent the FCNJ program at community outreach events
  • Assist with mentoring, precepting, and conducting shadow visits for new nursing staff during orientation
  • Assess caregiver and newborn physical health, infant attachment, intimate partner violence, substance use disorders, perinatal mood disorders, and connection to health and community services.
  • Provide referrals to community services as needed
