Requirements
- Be currently licensed as a Registered Nurse (RN) in the state of Idaho
- Demonstrate knowledge and skill in nursing practice
- CPR professional certification
- General knowledge of computers and the ability to learn industry-specific software and documentation applications
- Possess a valid state driver’s license and automobile liability insurance
Nice To Haves
- A minimum of one year of clinical experience is preferred
Responsibilities
- Work with patients, their families, and other healthcare professionals in the patient’s place of residence
- Perform visits and complete coordination of care
- Act as the point of contact for all disciplines involved with providing care to the patients
- Follow agency, state, and federal policies and procedures
- Consult with the patient’s primary care physician
- Collect OASIS information (Home Health only)
- Ensure medication profiles are accurate
- Attend meetings as required
