The Nurse Family Partnership (NFP) Nurse Home Visitor (NHV) is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. This position establishes and maintains therapeutic relationships while delivering individualized client case management services. The NHV conducts home visits to women and their families eligible for the NFP Program to determine patient and family needs, adhering to the nursing process and the NFP model of home visitation. The role involves managing a caseload of up to 25 to 30 clients for a full-time NHV, maintaining current knowledge of agency programs and policies, and serving as a liaison or representative for the agency in community organizations. The position also assists with responses to Public Health emergencies and emergency preparedness exercises, and is responsible for maintaining a current nursing license and updating program information through continuing education.
