About The Position

Responsible for patient care and support within our hospital's outpatient setting through providing compassionate care, assisting in procedures, and collaborating effectively in a dynamic ambulatory care environment. The role involves delivering direct patient care, conducting assessments, devising care plans, educating patients and families, collaborating with healthcare professionals, administering medications, performing IV therapy, and maintaining accurate patient records. Upholding infection control protocols and maintaining a safe environment are also key responsibilities.

Requirements

Graduate of an approved school of nursing with current registration in Massachusetts.
Bachelor of Science Degree in Nursing is required for newly licensed nurses.
Minimum of 2 years of recent (within 12 months) clinical nursing experience in an Ambulatory Clinic/Practice setting required. Experience only in an ambulatory procedural area is not sufficient.
Experience providing in-person triage and direct patient care for acute and chronic conditions as appropriate for an ambulatory outpatient setting, urgent care, or ED required.
Experience with telephone triage and telephone-based clinical assessment techniques required.
Proficient knowledge of nursing principles, practices, and procedures.
Strong communication and interpersonal skills.
Ability to work effectively in a fast-paced and dynamic healthcare environment.
Demonstrated commitment to continuing education and professional development.
Familiarity with electronic medical records (EMRs) and healthcare technologies.

Nice To Haves

Experience with Endocrine, Renal and Gastroenterology-related patient care delivery as an RN strongly preferred.
Experience with team-based care preferred.
Case Management experience preferred.

Responsibilities

Deliver direct patient care and support in our ambulatory care unit, ensuring high nursing standards.
Conduct patient assessments, devise care plans, and assist in outpatient procedures with a focus on patient comfort and safety.
Educate patients and families on treatment plans, medications, and post-procedure care.
Collaborate closely with healthcare professionals to coordinate comprehensive patient care.
Administer medications, perform IV therapy, and maintain accurate patient records in compliance with regulations.
Uphold infection control protocols and maintain a safe environment for patients and staff.
Accountable for assessing, planning, implementing and evaluating a plan of care for a specific patient assignment.
Sets priorities when organizing care for patients with varying acuity.
Maintain continuity through clear and concise (verbal and written) communication.
Demonstrates appropriate knowledge of growth and development of the adult and geriatric patient.
Documents patient care in a manner that is clear, complete, concise and in compliance with nursing documentation standards.
Develops a comprehensive plan of care based on data from an initial assessment of patient and family, information from other members of the health care team, intra-agency referral and previous medical records.
Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources and documents according to the department’s documentation standards.
Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.
Evaluates the effectiveness of the plan of care, and documents progress in meeting stated goals. Revises plan of care as needed to achieve desired outcomes.
Participates actively in the unit-based and organizational quality management and/or quality improvement programs.
Provides a safe environment for patients, staff, family and visitors.
Administers medication safely according to established policies and procedures.
Performs nursing procedures safely and efficiently.
Uses equipment safely and efficiently.
Demonstrates awareness of potential/actual risks of infection and modes of transmission.
Utilizes universal precautions in nursing practice.
Interacts with patients, families, and colleagues in a professional manner.
