About The Position

Provides education, experiences, and care coordination services for pediatric and young adult patients and their families. Develops and implements educational programs for patients, their families, hospital staff, and community health personnel. Maintains flexibility working in the inpatient, clinic, and office settings. May require working in clinics off site from main campus.

Requirements

  • BSN, required.
  • Registered Nurse with current licensure to practice nursing in the State of Ohio required.
  • 3 years of experience as an RN required.
  • Effective customer service and communication skills (oral, written, presentation).
  • Ability to organize priorities when competing demands occur and work within a fast-paced environment with timeframe and deadline expectations.
  • Competency providing education in individual and group settings.
  • Provision of culturally competent and equitable care.

Nice To Haves

  • pediatric experience and/or diabetes education preferred
  • Certified Diabetes Care and Education Specialist (CDCES) within 2 years of hire. Resources provided.

Responsibilities

  • Educates in a patient and family centered approach, including assessment, intervention, and evaluation.
  • Develops and maintains effective working relationships among health care professionals who provide care to patients with diabetes.
  • Collaborates with service providers to identify patients in need of care coordination.
  • Works closely with families to guide through the health care system, optimize care and services, and improve child health and satisfaction.
  • Demonstrates continuous learning to keep current with changes in diabetes management and technology.
  • Participates in development of educational materials and updates for patients and families.
  • Provides individual and classroom style education, targeting needs of patients and other entities, such as school or group home staff.
  • Participates in telephone triage by answering live calls from families and other entities to address questions and concerns.
  • Collaborates with the diabetes team and networks with internal and external stakeholders, including staff from schools and residential programs.
  • Engages in “workplace community” by staying aware of changes, participating in data collection, auditing, and quality improvement, and represents the team at relevant meetings.
  • Reports information and updates back to the team.
