About The Position

Performs patient assessment and plans, delivers and evaluates patient education to selected populations across the continuums of illness and lifespan. Assumes a role as a resource person by participating in community outreach and education support.

Requirements

  • Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Obtain Certified Diabetes Care and Education Specialist (CDCES) Certification through the Certification Board for Diabetes Care and Education within two (2) years of hire date.
  • Obtain certification in Basic Life Support within 30 days of hire date.
  • Two (2) years of clinical experience with One (1) year in the care of diabetic patients.
  • Knowledge of relevant scientific principles, established standards of care and/or research findings.
  • Knowledge of procedures and techniques involved in administering routine and special treatments to patients.
  • Knowledge of and appropriate application of the nursing process.
  • Knowledge of professional theory, practice and procedure.
  • Ability to communicate clearly with patients, families, physicians, and other employees.

Nice To Haves

  • Bachelor’s degree in Nursing (BSN).
  • Certification in Diabetic Education upon hire.
  • Five (5) years of clinical experience in nursing.

Responsibilities

  • Identifies education needs of the community and those with diabetes.
  • Attends diabetes advisory committee meetings as assigned.
  • Performs patient assessment, delivers education and evaluates patient’s obtainment of goals.
  • Assists in maintaining the American Diabetes Association certification as applicable.
  • Participates in community outreach and educational support.
  • Advocates on behalf of the patient and families.
  • Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes.
  • Participates in performance improvement activities to assure quality patient care.
  • Completes daily rounds to assess patient status.
  • Ensures all necessary testing and scheduling is complete prior to necessary procedures.
  • Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.
  • Leads the planning, implementation and evaluation of the Outpatient Diabetes and Nutrition Education Program.
