About The Position

This position is located at the Lower Brule Health Center in Lower Brule, SD. The primary purpose of this position is to provide professional community/public health nursing services through direct care to individuals, families, and identified populations through direct care, home and school visiting, and population-based assessment, prevention, and intervention activities. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

  • This is a designated position covered by Public Law 101-630, requiring contact or control over Indian children. Due to this requirement, the agency must ensure that persons hired for these positions have not been found guilty of or pleaded nolo contendere or guilty to certain crimes.
  • The selected individual is required to obtain and maintain any licensure requirements. If licensure are not obtained or maintained during employment, the employee will be subject to adverse actions, up to and including removal from Federal service.

  • Background Investigation Requirement: This position requires the successful completion of a federal background investigation as a condition of employment.
  • Financial Suitability Requirement: This position requires a review of the applicant's financial history as part of the personnel security and suitability determination.
  • Prior to the appointment, candidates must present two valid forms of identification. If one of the documents is a state-issued ID or driver's license, it must be REAL ID-compliant.
  • A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required.
  • Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
  • Applicants must have passed the National Council Licensure Examination.
  • Possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
  • Applicants must successfully complete a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; -OR- Have possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program and at least one (1) full year of professional nursing experience (equivalent to the GS9 grade level) in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Performing chart reviews against established or self-developed criteria, such as Community Health Nursing quality measures, provide primary health care a generalized public health needs of a community and develop a health profile; independently plan for , coordinating, managing and evaluating a community health nursing program, clinic or unit including delegating work and leading others.

Responsibilities

  • Assesses the health needs of individuals, families, and the community served, taking into account the total situation in the family, the home, and in the community environment.
  • Develop individualized and family-centered care plans.
  • Implements and evaluates plans of care to support prevention, early intervention, treatment adherence, and continuity of care.
  • Shares health information and collaborates with patients, families and other community resources.
  • Education and coaches people to make appropriate and informed decisions about health issues.
  • Apply population health assessment and surveillance findings to support targeted prevention, early intervention, and public health nursing activities within the assigned service unit.
