University of Iowa Healthcare Department of Nursing is seeking a NICU Little Hero Continuity of Care Nurse Clinician to perform specific tasks to assist members of the Nursing team in providing patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division. This role involves planning, monitoring, and documenting patient care across the continuum from inpatient and outpatient services, educating patients and families about their treatment plans, and serving as a patient advocate. The clinician will also facilitate the assessment of patient needs, complete community referrals upon discharge, and observe for breakdowns in the plan of care. Collaboration with external healthcare providers is essential for smooth transfers, and the role includes identifying performance opportunities and assisting with improvement plans for specific programs or populations. The Nurse Clinician will monitor continuity of care, coordinate planning for Iowa’s Medical Home and Community-Based Waiver programs, and provide follow-up calls to families of children with complex healthcare needs. Responsibilities also include coordinating and documenting post-discharge communication, acting as a liaison between families, insurance companies, and community providers to secure necessary services, and assisting with special healthcare needs and related program development. The position is 70% time, scheduled for 28 hours per week between Monday and Friday, 0800-1630, with no weekends or holidays. The location is Stead Family Children's Hospital (SFCH).
