About The Position

Activa Home Health (an Accord Care Company) is seeking an enthusiastic, customer service-driven RN Clinical Manager for our Broward office. This position will primarily work in the office and will also perform field case management with responsibilities for assessment, coordination of care with other disciplines, and communication with referral sources. The role involves coordinating patient admissions with other members of our care team. A strong background in case management and working knowledge of Medicare are essential.

Requirements

• Current RN Florida professional license.
• Minimum of one year of experience with Medicare home health case management.
• Excellent time management skills to prioritize and organize workload.
• Positive attitude and strong work ethic.
• Excellent communication and customer service skills.
• NEGATIVE TB Test / NEGATIVE QuantiFERON Blood Test.
• Must pass a screening through the Care Provider Background Screening Clearinghouse.

Nice To Haves

• Experience with Kinser Wellsky.

Responsibilities

• Perform case management oversight and coordination of care for all skilled disciplines.
• Maintain close communication with the social worker and veterans division.
• Coordinate patient admission with other members of the healthcare team.
• Provide nursing care and treatment of home care patients in accordance with the plan of treatments and approved authorizations.
• Supervise all non-skilled caregivers.
