About The Position

Reporting to the Nursing Director, the Registered Nurse/Licensed Practical Nurse (RN/LPN) Nurse Case Manager will be accountable for the full patient care cycle while the child is in foster care. The successful candidate will be responsible for coordinating children and youth’s medical and mental health care and providing children and youth with resources to foster continued good lifetime practices needed for independent living. The NCM encourages good health practices and preventative healthcare with foster parents and children/youth. This includes developing, implementing, and evaluating individualized patient care plans for children on their caseload. The nurse serves as an advocate and liaison for the children (age range from birth to 21 years old) and ensures the wellbeing of the individual child/youth.

Requirements

  • Valid and unrestricted nursing license
  • Bachelor of Science in nursing (BSN), who is also a registered nurse, registered nurse (RN), RN or licensed practical nurse (LPN)
  • Certification by the American Red Cross (or an equivalent) in BLS with First Aid or the ability to obtain before hire
  • Two years of work experience, resumes will also be reviewed for related work experience if there is a shorter timeframe of work experience

Responsibilities

  • Assesses, plans, implements and monitors the overall healthcare of the foster child and coordinates and provides care promptly by child developmental milestones and as needed
  • Ability to establish effective working relationships with birth parents, foster parents, casework staff, schools, community providers and all members of the child's care team to ensure the safety and overall optimal healthcare of children in foster care
  • Schedules required medical appointments and specialty appointments within requested and mandated timeframes, conducts assessments, including but not limited to developmental, HIV risk assessments, intake assessments, general first aid and triage activities
  • Conducts routine screening for substance abuse, developmental health, mental health and physical body checks for abuse
  • Ability to identify physical signs and indicators of physical abuse. Ability to identify possible indicators of sexual abuse
  • Participates in all on-site well-child clinics on scheduled days including documentation of vital signs, foster parent and child/youth education and assisting agency pediatrician
  • Provides routine management, training, teaching and health education including reproductive health education for children and youth
  • Provides teaching, education, and management for chronic conditions, such as diabetes and asthma
  • Provides education and training for foster parents regarding chronic conditions
  • Provides support and management of the emotional/psychiatric needs of children
  • Establishes comprehensive treatment goals for children, utilizing historical and currently available information in collaboration with the multi-disciplinary team
  • Timely follow up after medical appointments including obtaining medical reports and follow up within 24 hours for urgent/emergency care or hospitalization
  • Maintains proper medical documentation including but not limited to timely and accurate medical progress notes
  • As needed, assists with home visits, community provider visits
  • Timely communication with child/youth’s treatment team including medical, case planning, mental health, external providers and other specialties involved
  • Ensuring all emails and phone calls are answered by EOB
  • Follows all requirements provided by regulatory bodies such as ACS and OCFS
  • Assists with maintaining medical supplies and equipment
  • Practices proper infection control
  • Proficient with technology including but not limited to Microsoft software
  • Experience at a demonstrated competent level as well as ability to learn/use internal EHR and Connections
  • Other relevant duties as assigned
