Nurse Case Manager - Atlanta
About The Position
Paradigm is seeking a Field Nurse Case Manager to cover the Atlanta area, including but not limited to Athens, Gainesville, Lawrenceville, Douglasville, and McDonough. This is a hybrid position requiring the candidate to be located in the Atlanta area due to regular local travel (80% of the time) for in-person patient appointments. The individual will be responsible for the medical case management of work-related injuries, involving assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager will collaborate with insurance carriers, medical care providers, attorneys, employers, and employees, closely monitoring the progress of injured workers and reporting results. Candidates must be willing to learn and manage catastrophic cases.
Requirements
- Current Registered Nurse (RN) license in GA
- CCM, CRRN, COHN, or CDMS required
- Valid Driver’s License, and good driving history
- Located in the Atlanta area
- Willing to travel 80% of the time for in-person patient appointments
- Organization and timeline adherence skills
- PC and technology skills
- Communication skills (written, verbal, and interpersonal)
Nice To Haves
- GA Rehabilitation Supplier preferred, required within one week of start date
- Experience in workers compensation case management
Responsibilities
- Medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals.
- Work with insurance carriers, medical care providers, attorneys, employers, and employees.
- Closely monitor the progress of the injured worker and report results back to the employer and insurance carriers.
