About The Position

The Nurse Case Manager will be responsible for assisting with planning and leading health-related activities. The ideal candidate will be sensitive to the needs of individuals with memory loss and elders from all socioeconomic backgrounds, and possess the demonstrated ability to work cooperatively as a multi-disciplinary team member. This role involves both clinical/nursing responsibilities and case management/social service responsibilities to ensure comprehensive participant care.

Requirements

  • Licensed to practice in Massachusetts.
  • Bachelor's degree in human services or a related field from an accredited college or university.
  • At least one year of relevant and recent experience working with adults in a professional capacity required for case management.
  • Minimum of one-year direct service experience with the elderly or the chronically disabled.
  • Minimum of one-year direct service experience working with individuals with memory loss and/or Dementia.
  • Demonstrated ability to assist with planning and leading health-related activities.
  • Strong communication, organizational and documentation skills.
  • Must be sensitive to the needs of memory loss, elders of all socioeconomic backgrounds and have the demonstrated ability to work cooperatively as a multi-disciplinary team member.

Responsibilities

  • Develop, monitor, implement and evaluate the treatment and care plans for each participant.
  • Participate in the ongoing monitoring of each participant's health status through direct contact with physicians, other nurses, health professionals and family members.
  • Carry out the physician's orders for each participant, including monitoring vital signs, administering medications, monitoring blood sugar, and implementing treatments. Coordinate these orders with family and/or other health care providers. Maintain good follow-up communication with caregivers and physicians.
  • Implement and supervise restorative therapy treatments as recommended by physical, speech, and/or occupational therapists.
  • Assist, monitor and instruct participants, program assistants and all staff who assist with personal care on matters of personal hygiene, the use of special aids, accident prevention, and ambulation and transferring skills.
  • Supervise program assistants that provide personal care and other daily tasks.
  • Provide health education/training on an individual and group basis, as well as during scheduled monthly clinical groups.
  • In conjunction with Program Manager responsible for documentation of initial health assessment, care plans, monthly nursing notes, MDS and reassessment forms.
  • Serve as primary advocate for participants and caregivers, coordinating services to support social, emotional, and health-related needs.
  • Develop, implement, and update individualized social service care plans, progress notes, and discharge plans as required by regulations.
  • Coordinate services with other agencies, caregivers, and health providers to ensure continuity of care.
  • Conduct family and care planning meetings; participate in interdisciplinary and clinical meetings.
  • Provide counseling, crisis intervention, and behavioral support as needed.
  • Assist participants and families in accessing benefits and community resources when no other advocate is available.
  • Lead or co-lead group discussions, caregiver education, and support sessions.
  • Assist with admission functions including referral management, tours, guest days, and eligibility assessments in collaboration with nursing staff.
  • Participate in interdisciplinary case conferences and weekly clinical meetings.
  • Assist participants as needed with Activities of Daily Living (ADLs) in accordance with care plans.
  • Maintain current knowledge of community resources and regulatory requirements.
  • Ensure all care and documentation meets DPH and DMA standards.
  • Work collaboratively with the Program Director and team to ensure quality of care, participant satisfaction, and program compliance.
  • Perform other related duties as assigned to meet program needs.
