About The Position

FlexStaff is hiring a part-time temporary Nurse Case Manager to join our client's team. The schedule will vary each week, with 3 days per week coordinated between the site and employee. The hours will be 9 AM to 5 PM. The assignment length is 13 weeks. The caseload is 25-30 people, with the number of houses varying depending on the number of residents (4-14 people per house). The Nurse Case Manager must visit each house they cover a minimum of once per week. There is a possibility of covering additional houses if another nurse is out. The role involves coordinating the delivery of health care services for assigned persons receiving services in a residence and/or program unit, making nursing assessments and diagnoses, and working with the Interdisciplinary Team (IDT) to achieve positive health outcomes in compliance with OPWDD and applicable regulations.

Requirements

  • Maintain current NYS Registered Nurse license.
  • Graduation from an accredited nursing program.
  • Excellent oral and written communication skills.
  • Competence in computer skills - Microsoft Office.
  • Ability to travel to and access multiple locations throughout agency locations.
  • Flexible work schedule with hours and/or days – as approved by the Director of Nursing.
  • Perform all job responsibilities in full compliance with all applicable laws, rules, regulations, policies, and procedures.
  • Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal False Claims Act, whistleblower protection, and mandatory reporting.

Nice To Haves

  • Bachelor’s Degree preferred.
  • Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
  • Electronic medical record experience preferred.

Responsibilities

  • Coordinate the delivery of health care services for persons receiving services as assigned in a residence and or program unit.
  • Make nursing assessments and diagnoses.
  • Work in conjunction with other members of the Interdisciplinary Team (IDT) to achieve positive health outcomes for persons receiving services in compliance with OPWDD and applicable regulations.
  • Travel to and access multiple locations throughout agency locations.
  • Complete corporate compliance training at least annually.
  • Fully cooperate in all corporate compliance investigations and reviews.
