This posting is intended to fill an existing vacancy for our Nurse Case Manager role. The Nurse Case Manager will provide support to patients and healthcare providers by offering information on Patient Support services, specific diseases, and therapies. They will act as a liaison between patients, families, and healthcare providers regarding medication access, address general inquiries, and offer continuous support to ensure patient adherence. The role involves documenting patient information, conducting file reviews, following case-management protocols, and collaborating with reimbursement specialists and physicians to secure funding for therapies. The Nurse Case Manager will also coordinate with healthcare providers, pharmacies, and clinics for treatment logistics, maintain communication with management, and assist in training new Nurse Case Managers. Reporting Adverse Events and other assigned duties are also part of the role, with potential standby/on-call responsibilities.
