About The Position

The Nurse Case Manager (NCM) is a non-clinical role focused on assisting service members in achieving full medical and dental readiness for the Unit Manning Roster (UMR) and Deployment Manning Document (DMD). The NCM acts as a liaison between the unit of assignment and the Office of the State Surgeon, ensuring accurate medical documentation and readiness tracking.

Requirements

  • Bachelor of Science in Nursing.
  • Three (3) years of Supervisory Experience.
  • Current, active, and unrestricted license to practice nursing in Arizona.
  • Certification in Cardio-Pulmonary Resuscitation (CPR) for Healthcare Providers by the American Heart Association.
  • Must be a U.S. Citizen.
  • National Background Check with Inquiries and Fingerprints, and Drug Screen.

Responsibilities

  • Apply current medical policy and render intake case management services at Periodic Health Assessment (PHA) and Performance Requirements Summary (SRP) to identify service members' individual medical needs.
  • Ensure the Total Force Health Readiness Flowsheet [DD Form 2766] is complete and accurate, and create Medically Non-Deployable (MND) referrals in E-Case to chart all medical correspondence.
  • Collect all necessary medical documentation to substantiate a service member's deployment status, ensuring it is placed in their individual medical record and scanned into their electronic record.
  • Ensure MEDPROS accurately reflects the service member's status and generate provider review packets for clearing/profiling upon receipt of appropriate medical documentation.
  • Ensure the provider review packet is reviewed in a timely manner and deliver the provider's disposition to the unit of assignment and the appointed medical representative.
  • Forward disposition and medical record to Health Systems Section (HSS) for board processing if applicable.
  • Ensure all administrative charting in eCase, Dental Classification Module (DENCLASS), Medical Data Protection System (MEDPROS), and eProfile, Medical Health Assessment/Periodic Health Assessment Medical Health Assessment/Periodic Health (MHA/PHA) is completed in a timely manner.
  • Ensure Medical Electronic Data Care History and Readiness Tracking (MEDCHART), Automated Voucher System (AVS), Armed Forces Health Longitudinal Technology (AHLTA), Medical Operational Data System (MODS), and Medpros Web Data Entry (MWDE) are completed in a timely manner.
