About The Position

This position is part of the Department of Disabilities, Aging and Independent Living (DAIL) Adult Services Division, within the Choices for Care program. The program helps older Vermonters and people with disabilities receive long-term care services and supports in the setting of their choice. The Nurse Case Manager/Utilization Review Nurse role involves performing in-person/virtual clinical eligibility assessments and utilization review for Vermont's Long-term Care Medicaid Choices for Care program, Brian Injury program, Attendant Services program, and the Adult High Technology Program. This position is based in Burlington and requires regular travel to neighboring counties. Work is performed under the general supervision of the Nurse Administrators and program manager.

Requirements

  • Possession of or eligible for licensure as a Registered Nurse RN in Vermont OR eligible to practice in the state of Vermont via a multi-state license
  • Three (3) years or more of professional nursing experience in an acute hospital setting, long term care, health insurance carrier, or within a community health/public health setting.
  • Must maintain Vermont licensure as a Registered Nurse OR eligible to practice in the state of Vermont via a multi-state license as a condition of employment.
  • Reliable means of personal transportation is required for work out of the office or in the community and field based settings.
  • Candidates must pass any level of background investigation applicable to the position.
  • Fingerprint supported criminal record checks, adult and child abuse registry checks, the Office of Inspector General exclusion database, and motor vehicle driving record checks, as appropriate to the position under recruitment, will be conducted on candidates, with the exception of those who are current classified state employees seeking transfer, promotion, or demotion into a classified position or are persons exercising re-employment RIF rights.
  • For positions at the Vermont Veterans' Home: Any person who has had a charge of abuse, neglect or exploitation substantiated against him or her OR who has been convicted of an offense for actions related to bodily injury, theft or misuse of funds or property is not eligible under the Vermont Veterans' Home licensing requirements.
