Requirements

Active Florida RN license and current BLS.
2+ years of pediatric nursing experience, with significant time in high-acuity or complex care.
2+ years of home health experience.
2+ years of case management or care coordination experience.
Working knowledge of Medicaid, managed care, and pediatric authorizations.
Proficiency in EMR, basic spreadsheets, and messaging tools.
Valid driver's license and reliable transportation for local visits in Gainesville.

Responsibilities

Manage a caseload of medically complex pediatric patients in Gainesville.
Build and update care plans that reflect actual home situations.
Handle prior authorizations and renewals to ensure continuous coverage.
Facilitate smooth transitions from hospital discharges to home care plans.
Educate parents and caregivers on managing medications, equipment (pumps, vents), and feeding tubes.
Coordinate communication between physicians, payers, supply companies, and field nurses.
Support field nurses with challenging cases.
Maintain clean, timely, and audit-ready documentation.
