Requirements

Active Florida RN license and current BLS.
2+ years of pediatric nursing, with significant experience in high-acuity or complex care.
2+ years in home health.
2+ years of case management or care coordination.
Working knowledge of Medicaid, managed care, and pediatric authorizations.
Proficiency with EMR, basic spreadsheets, and messaging tools.
Spanish fluency is required.
Valid driver's license and reliable transportation for local visits across Miami.

Responsibilities

Manage a caseload of medically complex pediatric patients in Miami.
Build and update care plans that reflect actual home situations.
Handle prior authorizations and renewals to ensure continuous coverage.
Develop clear discharge plans from hospitals for families and field nurses.
Educate parents and caregivers on managing medications, pumps, vents, and feeding tubes.
Coordinate communication between physicians, payers, supply companies, and field nurses.
Support field nurses with challenging cases.
Maintain clean, timely, and audit-ready documentation.
