About The Position

The Nurse Case Manager II reports to the Manager of Case Management and Care Transitions and works under the guidance of the Nurse Case Manager Team Leader as a member of the clinical team. The role coordinates and manages a plan of care across the care continuum to achieve clinical outcomes, promote safe and timely discharge, and support efficient patient movement through the hospital system. The Nurse Case Manager II serves as a clinical expert in discharge planning, resource utilization, and case management.

The position is scheduled for 40 hours per week, Monday through Friday on day shifts, with occasional weekends and holidays and no nights. The position is salaried.

Requirements

• Bachelor's degree in Nursing from an accredited school of nursing.
• Three years of work experience as a Registered Nurse.
• Licensure as a registered nurse by the state of South Carolina or a compact state.
• Current American Heart Association Basic Life Support certification or American Red Cross BLS for Healthcare Providers certification.

Nice To Haves

• Prior leadership experience.
• One year of Case Management experience.
• Case Management certification.
