About The Position

The Nurse/Care Manager Level 2 position is part of the Flexible Assertive Community Treatment (FACT) Team. This role involves working as part of a care coordination team to provide comprehensive services to individuals, including health assessments, medication education, case management, and linkage to community resources. The position requires adherence to the ACT Treatment model and maintaining health records according to standards and regulations. The Nurse Care Manager will also act as a medical resource consultant and promote health and wellness activities.

Requirements

  • NYS Registered Nurse License
  • Two years’ professional experience in Mental Health required
  • Excellent interpersonal, organizational and communication skills
  • Ability to instruct staff and individuals in basic health care
  • Ability to work with the Interdisciplinary Treatment Team
  • Familiar with community agencies and resources.
  • Familiar with computer programs and Health Information Technology. (e.g. Word, Excel, EHR)
  • Ability to legally operate a vehicle in NYS
  • The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighting up to 40 lbs.

Nice To Haves

  • BSN preferred
  • Experience with dually diagnosed individuals preferred.

Responsibilities

  • Work as part of a Care Coordination team.
  • Conducts assessments on basic health and medical needs.
  • Provides ACT Treatment model which includes but is not limited to: Motivational Interviewing (Stages of Change) Dually Diagnosed Treatment, Wellness Management, Family Intervention, Cognitive-Behavioral Therapy, supportive counseling and case management.
  • Provide medication education to staff and individuals.
  • Participate in multidisciplinary team meetings and case reviews.
  • Identify and advocate for individual rights.
  • Teach/assist in developing/maintaining ADL skills.
  • Conduct initial and ongoing assessments.
  • Monitor routine health care of all individuals.
  • Provide/arrange for routine and emergency healthcare.
  • Review all health-related incidents.
  • Maintain health records according to standards and regulations.
  • Provide management and administration of medication in conjunction with psychiatrist and/or NPP.
  • Maintain/coordinate effective and on-going communication with other service providers.
  • Participate in the development/documentation/review and update of care plan.
  • Act as linkage to community services.
  • Coordinate and conduct care planning activities for a designated case load of recipients.
  • Utilize health information technology and electronic health record to coordinate care and communicate with providers.
  • Act as medical resource consultant for team members.
  • Promote health and wellness activities for recipients via individual or group sessions.
  • Provide crisis intervention services as necessary.
  • Provide transportation as needed to necessary services.
  • Promote self-advocacy and personal empowerment.
  • May be assigned to participate in rotating afterhours/weekend on call coverage.
  • Establish relationship with all individuals, family members, and advocates.
  • Provide medically related training to program staff as needed.
  • Understand and follow all ACT program policies and procedures.
  • Represent the agency and program in the community and with other service providers.
  • Participate in and successfully complete all required staff training and development activities.
  • Regular and substantial contact with individuals receiving services.
  • Perform other related tasks as directed by supervisor.
