Assertive Community Treatment (ACT) Registered Nurse (3820)
About The Position
Carolina Outreach is seeking a compassionate, dedicated, and skilled Psychiatric Nurse (RN) to join our Assertive Community Treatment (ACT) Team. In this role, you will play a hands-on part in transforming lives by delivering critical, community-based mental health services to individuals with complex needs. As an ACT Nurse, you will be at the heart of service delivery, partnering closely with clients, families, and an interdisciplinary team to ensure that every individual receives the care and support they need. You will oversee the medical and clinical components of treatment, blending your clinical expertise with a deep commitment to person-centered care. Requirements
- Registered Nurse must be licensed to practice in NC by the Board of Nursing.
- A valid driver’s license and current automobile insurance is also required.
- Each ACT team member must complete DMH-approved ACTT training.
- Must demonstrate competence in providing ACT services prior to service provision.
- Must be authorized to work in the United States.
Responsibilities
- Monitor and oversee all medical/clinical aspects of services delivered.
- Provide medication monitoring and documentation.
- Deliver psychopharmacologic and medical services within your scope of practice.
- Collaborate with ACT team members to monitor clinical status and response to treatment.
- Conduct crisis assessments, symptom management, and intervention planning.
- Provide individual counseling, substance abuse treatment, and psycho-educational support.
- Assist in creating transition plans when clients move in and out of services.
