About The Position

Since 2003, Carolina Outreach has been assisting children, families, and adults across North Carolina with their behavioral health needs. We are a group of highly trained behavioral health professionals dedicated to promoting the emotional and physical well-being of our community in a safe and respectful environment. We provide a comprehensive and integrative array of behavioral health services and supports for people of all ages with varying levels of need utilizing an equitable and inclusive approach. We focus on helping our clients tap into and develop their inherent strengths with the goal of supporting the highest quality of life possible for all we serve. We are seeking a full-time Mental Health Therapist to provide community-based, in-home individual and family interventions to Mental Health and Substance Use Disorder clients receiving ACT team services. The person in this position will work with clients, families, and natural supports to provide service coordination, crisis assessment and intervention with ACT consumers.

Requirements

  • Master's Degree in counseling, social work or related field
  • Licensed in NC as a LCSW/A or LCMHC/A

Responsibilities

  • Provide individual, family, and/or group therapy sessions
  • Maintain accurate and timely clinical documentation
  • Collaborate with care teams as needed
  • Maintain compliance with all clinical and regulatory standards
  • Participate in supervision (if applicable based on licensure level)
  • Provide service coordination
  • Provide crisis assessment and intervention
  • Provide symptom assessment and management
  • Provide individual counseling and psychotherapy
  • Provide substance abuse treatment
  • Provide work-related services
  • Provide activities of daily living services
  • Provide social, interpersonal relationship, and leisure-time activity services
  • Provide support services or direct assistance to ensure that individuals obtain the basic necessities of daily life
  • Provide education, support, and consultation
  • Provide mental health assessments and recommend/implement treatment options based on these assessments
  • Provide clinical guidance to the team about clients’ mental health issues
  • Develop and implement (or assist in the development and implementation of) clients’ person-centered plans
  • Provide clinical guidance and/or oversight in the development of crisis plans and transition plans when clients move in and out of services
  • Implement therapeutic interventions, crisis interventions, behavioral interventions, psycho-educational activities, and other best evidence-based practice therapies
  • Work with clients, families, natural supports, and collateral agencies in reviewing, applying, and achieving goals outlined in the person-centered plan
