About The Position
KenCrest is seeking Nurse Care Managers (RN or LPN) for our Residential Program to manage the healthcare needs of our medically fragile individuals with intellectual disabilities in Wester Montgomery County, PA. As a Nurse Care Manager, you will coordinate, prepare, and attend all Resident medical appointments, collaborate with Care Teams for best care deliveries and reintegration into Community Homes, conduct bi-weekly home visits of all assigned persons who reside in a KenCrest Community Home, and complete required documentation, assessments and evaluations as needed.
Requirements
- Valid RN/LPN License in the state of PA
- Valid Driver's License
- Experience working with individuals with intellectual disabilities
- Current Healthcare Provider CPR
- Clear criminal record and other background checks in accordance with state requirements
Responsibilities
