As part of the CCBHC’s multidisciplinary team, the Nurse Care Manager will assess individuals/families with behavioral health and/or substance abuse issues and provide treatment and referral services. The Nurse Care Manager will lead the development and ongoing management of a culturally competent, comprehensive, individualized care plan with each patient.
