About The Position

The Nurse Care Coordinator is responsible for the 24-hour management of nursing care delivered to residents on assigned unit. This position is responsible for supervising, directing, and evaluating the assigned nursing personnel on the unit, ensuring nursing service personnel assigned on the unit demonstrate acceptable standards of practice and skills, assist in orientation, training and referral for further training, and recommends disciplinary action of subordinates.

Requirements

A Bachelor’s degree in Nursing or equivalent
A minimum of three years’ experience as a registered nurse
Previous experience as a nurse manager is required.
A current Wisconsin nursing license.

Responsibilities

Respond to complex nursing care situations and document assessment, intervention, and follow up to situations as necessary.
Monitor that medications and treatments are administered and completed safely and accurately according to physician order and standards of practice.
Assist staff nurses in assessment, nursing diagnosis, care plan development, implementation of interventions, and evaluation of the nursing plan of care.
Provide support for staff nurses regarding assessments of resident signs and symptoms and overall status, and intervene according to established facility policies and procedures if changes are noted, and document per facility standards.
Collaborate with staff nurses in observing changes in a resident’s status, assess for the need of further intervention, notify the MD if needed and develop, implement, adjust, and evaluate the nursing plan of care.
Consult with the physician to influence the medical plan of care, obtain physician orders, clarify orders, or inform regarding resident status.
Ensure quality of care is being provided to residents at all times.
Coordinate exceptional nursing care for residents in a professional manor.
Coordinate and function as a team member in resident care conferences with family, agencies, and departments as necessary.
Coordinate the nursing plan of care as an integrated part of the interdisciplinary plan of care and direct nurses to monitor nursing care plan status for accuracy in reflecting the current needs of the resident.
Participate in quality assurance programs to insure safety polices and emergency policies of the facility are followed, infection control practices are maintained, and resident rights are respected by all employees.
Assist in insuring that all facility and county personnel policies, nursing service policies and procedures, and standards of nursing practice are demonstrated by subordinates.
Participate in approved continuing education programs to maintain competence.
Establish a pleasant, cooperative and professional working environment within Nursing Service.
Delegate assignments, supervise, and evaluate professional and non-professional nursing service. personnel on all shifts.
Recommend staff transfers, disciplinary action, suspension or termination.
Receive employee complaints and make recommendations regarding adjustment of grievances.
Identify learning needs of individuals and implement training or refer for further training.
Adjust work schedule as necessary to assess and problem solve resident care and personnel issues on all shifts of assigned unit.
Work scheduled shift and demonstrate flexibility in assignment when changes are necessary.
Establish appropriate working relationships with co-workers, subordinates, and supervisors.
