About The Position

This role offers a stable, office-based schedule for a Registered Nurse (RN) within the Behavioral Health Home (BHH) program. It is designed for individuals seeking to transition from traditional hospital shifts to a more balanced work environment. The position involves a team-based approach to support clients with severe mental illness and chronic physical health conditions. Key functions include medical consultation, interdisciplinary collaboration, health education, and client advocacy. The schedule is part-time, 20-24 hours per week, during Monday-Friday business hours, with no nights, weekends, or on-call duties. The role emphasizes bridging the gap between physical and behavioral healthcare, empowering clients, and coordinating complex care without the physical demands of bedside nursing. Requirements

  • Active, unencumbered Registered Nurse (RN) license in the State of Maine (or valid multi-state compact license).
  • Current CPR/Basic Life Support (BLS) certification.
  • Valid driver's license, a good driving record, and reliable transportation.
  • Comfort navigating Electronic Health Records (EHR) and standard office software.
  • Exceptional communication and organizational skills.
  • Proven ability to work autonomously while seamlessly collaborating with a multidisciplinary team.
  • Successful completion of all required background checks; must not be listed on state or federal suspension and disbarment lists (OIG).

Nice To Haves

  • Prior experience in behavioral health, community health, or working with underserved, transient populations managing both clinical and psychiatric comorbidities.

Responsibilities

  • Provide occasional in-person advocacy, support, and medical care coordination for clients during appointments at local doctors' offices.
  • Serve as a primary medical resource for BHH Care Coordinators and Clinical Coordinators, integrating physical health strategies into behavioral health care plans.
  • Provide remote and in-person consultative support for clients transitioning out of hospitals, emergency rooms, or treatment facilities to ensure safe and successful discharges.
  • Conduct health education sessions for clients and offer health-literacy guidance to non-medical staff.
  • Utilize Electronic Health Record (EHR) to track client progress, analyze health outcomes, and translate medical data into actionable interdisciplinary care plans.
  • Advocate for client access to crucial external resources and coordinate with external community health providers.
  • Advise AWS management on health, safety, and infectious disease protocols, assisting in the development of internal policies.
  • Provide immediate clinical guidance and coordinate with emergency services during crisis situations.
