As a member of an interdisciplinary team and under the supervision of the Manager, the incumbent will be responsible for creating a therapeutic environment in which you will deliver a high standard of care to patients who are diagnosed with substance dependence and Mental Health concerns. The community Provincial Addictions Nurse Coordinator will provide intensive case management, physical assessment and substance withdrawal screening in order to recommend appropriate treatment options and develop a care plan. Additionally, mental health and suicide risk assessments will be required and safety planning for clients will be completed. As an Addictions nurse coordinator, naloxone training and formal community education will be required. The Addiction Nurse Coordinator will provide formal pharmacological education and addiction medicine education to Pharmacies, Labs, Emergency Departments, Physicians and SHA colleagues or community partners assisting with continuum of care for clients. Responsibilities include providing individual counselling which may include brief interventions, ongoing medication management counselling and crisis counselling. The Addiction Nurse Coordinator will assist clients with complex presentations, independently assess and determine and coordinate treatment while utilizing their clinical skills and expertise to direct unregulated health care workers and/or care providers. The incumbent will provide medication education to patients and assist Physicians in determining addiction medication options based on the Nurses assessment. The incumbent will perform other nursing duties as required.
