About The Position

Under the general supervision of the Manager, Health Services and while demonstrating a commitment to the mission, vision and values of the WRHA, the Rapid Response Nurse will support medically complex/frail adults within the Home Care Program identified as at risk for avoidable hospital readmission and/or Emergency Department visits. Rapid Response Nurses will provide short term targeted interventions involving comprehensive assessment, medication reconciliation, education on chronic disease management, review of discharge plans to ensure required follow-up and referral to community services and supports. Rapid Response Nurses will provide care both in the client’s home and by telephone follow-up and support. Rapid Response Nurses will work collaboratively with the client’s interprofessional team including but not limited to Emergency, Primary Care, Mental Health and other Home Care providers.

Requirements

  • Graduate of an approved Registered Nursing education program required.
  • Responsible for maintaining and providing proof of registration with the College of Registered Nurses of Manitoba (CRNM).
  • Cardiopulmonary Resuscitation (CPR) training requirements for this position will be in accordance with the Employer policy.
  • Valid Manitoba Class 5 driver's license required.
  • Knowledge of College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses.
  • Knowledge of Canadian Nurses Association Code of Ethics for Registered Nurses.
  • Knowledge of Community Health Nurse Standards of Practice & Home Health Competencies.
  • Knowledge of Scope of practice document as documented in the Regulated Health Professions Act (RHPA)/Registered Psychiatric Nurses Act.
  • Knowledge of Regional/Facility unit policies, procedures, protocols, and guidelines.
  • Knowledge of Nursing care related to community nursing.
  • Knowledge of Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices (Universal Precautions) and other legislated acts.
  • Knowledge of Roles and responsibilities of members of the healthcare team.
  • Knowledge of Principles standards of practice of telephone consultation; delegation/assignment.
  • Knowledge of Principles of adult learning and teaching strategies.
  • Knowledge of Principles of health behavior counselling and motivational interviewing.
  • Demonstrated effective oral and written communication skills.
  • Demonstrated critical thinking/problem-solving skills.
  • Ability to prioritize care for an individual or group(s).
  • Ability to delegate/assign care appropriately and monitor outcomes of delegated/assigned care.
  • Ability to respond to a variety of simultaneous demands.

Nice To Haves

  • 2 years related recent experience preferred.
  • Experience in emergency, medicine, rehabilitation and/or community an asset.
  • CNA Community Health Nurses Certification or relevant applicable certification preferred.
  • Additional chronic disease management education preferred.
  • Active participation in professional associations e.g. Association of Regulated Nurses of Manitoba (ARNM) preferred.
  • Preference will be given to those applicants competent in an Indigenous language and/or knowledge of Indigenous customs, traditions and values.

Responsibilities

Benefits

  • Pensionable Salary Enhancement - Nurses in eligible classifications may be eligible to earn an additional $5.95/hour when working full-time hours within a prescribed six-month reporting period. (Effective April 1, 2025 – March 31st 2027.)
