About The Position

Sharp Rees-Stealy Medical Group is seeking an experienced Family Nurse Practitioner or Physician Assistant to join our Sharp Extended Care Department. This is a 1.0 FTE full-time salaried position with a Monday-Friday schedule and one hour for lunch. This is a 100% outpatient position providing float coverage across Sharp Extended Care and Care at Home services. Sharp Extended Care involves managing post-acute patients in skilled nursing facilities, including patient rounding, care coordination, and discharge management. Care at Home involves conducting approximately 5 to 6 home visits per day, delivering intermittent skilled medical care to homebound patients in their residences. The role also includes leading advanced care planning conversations and collaborating closely with patients, families, and interdisciplinary care teams. No weekends or inpatient care are involved.

Requirements

Minimum of 2 years of full-time NP/PA experience in an outpatient setting.
Active, unrestricted California NP or PA license required.
Current board certification in applicable specialty required.
Must be comfortable working independently.
Excellent communication skills with the ability to lead goals-of-care and advanced care planning discussions with patients and families.

Nice To Haves

Experience in geriatrics, post-acute care, hospice, home-based care, or skilled nursing facility (SNF) settings strongly preferred.
