About The Position

Optum Care at Home - Thurston County, WA is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join their team. This is a full-time position focused on providing in-home care to patients. The role involves daily travel to patient homes within an assigned area. Optum Home & Community Care, part of Optum, is developing an integrated care model to address individuals' physical, mental, and social needs, helping patients access care anywhere, anytime. The Optum Care at Home program is a longitudinal, integrated care delivery program that coordinates clinical care for patients in their residences. The Advanced Practice Clinician (APC) within this program cares for high-risk health plan beneficiaries as part of an interdisciplinary team. Requirements

For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
For PAs: Current NCCPA certification and state licensure.
Active, unrestricted licensure in the state of practice (or ability to obtain by start date) without current disciplinary actions or practice limitations.
Active and unrestricted driver’s license.
Access to reliable transportation.
Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice).
Ability to gain a collaborative practice agreement, if applicable in your state.
Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties.

Nice To Haves

1+ years of clinical experience as an APC (family, geriatric, or home health preferred).
Experience in geriatric medicine, long-term care, senior living or home care setting.
Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs.
Understanding of Geriatrics and Chronic Illness.
Understanding of Advanced Illness and end of life discussions.
Proficiency with electronic medical records and technology.

Responsibilities

Conduct comprehensive in-home assessments, including past medical history review, medication reconciliation, vital signs, and physical exam.
Perform evidence-based screenings and point-of-care testing as appropriate.
Manage members through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
Manage members during short-term sub-acute rehabilitation episodes.
Ensure accurate and complete documentation, including ICD 10 conditions.
Communicate and collaborate with the interdisciplinary care team.
Conduct advanced illness and advanced care planning conversations.
Provide counseling and education to patients and caregivers.
