About The Position

Optum’s Senior Community Care team is hiring a Nurse Practitioner or Physician Assistant for a per diem, field-based role providing longitudinal care to medically complex patients in community-based residential settings throughout Marion County, Oregon. Settings include assisted living, independent living, adult family or foster homes, senior living apartments, and private homes. The clinician serves as the primary advanced practice clinician for a patient panel, makes clinical decisions in collaboration with patients’ primary care providers, and receives support from an interdisciplinary team. The value-based model emphasizes quality of care rather than patient volume or productivity quotas. The clinician will care for high-risk Medicare members with multiple comorbidities, advanced disease states, and complex treatment needs. Care addresses patients’ medical, behavioral, functional, and social needs in partnership with caregivers, facility teams, and other healthcare professionals. The schedule is flexible, with a minimum of 24 hours per month. No on-call, weekend, or holiday work is required. Optum Home & Community Care is part of the Optum family of businesses. Its Senior Community Care team provides support in nursing homes, senior housing, and assisted living settings to improve access to care and connect patients’ care across settings.
