The Nurse Practitioner in Pre Admission Testing performs comprehensive histories and physicals, assesses patients, and documents findings in the medical record. The role includes ordering and collecting relevant assessment and diagnostic information, managing diagnostic tests, formulating differential diagnoses, diagnosing complications, and ordering appropriate interventions. The APP may serve as a first responder when patient status changes or when procedural and resuscitative interventions are needed. The position assesses risks related to care of acute and complex chronically ill patients, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures. It develops, documents, implements, and modifies individualized plans of care using evidence-based guidelines and protocols, and may perform advanced procedures consistent with privileges and competency validation. The Nurse Practitioner documents patient progress and care in the medical record, communicates and collaborates with the interdisciplinary health care team, and supports continuity of care among patients, families, and staff across inpatient and outpatient settings. Professional practice includes precepting and mentoring, educating graduate students, engaging in research and evidence-based practice analysis, participating in Advance Practice Grand Rounds and specialty meetings, and maintaining continuing education and professional organization membership.
