About The Position

This position provides HIV non-medical case management to ensure Ryan White Part B clients enter and remain in HIV medical care and have access to and are adherent to their HIV medications. The role involves providing case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program, performing client intake, assisting with applications and re-certifications, assessing needs, developing care plans, coordinating care and benefits, providing and tracking referrals, offering internal and external resources, and performing follow-up as needed.

Requirements

  • One year of paid employment/professional experience providing case management services, excluding internships, AND a Bachelor of Social Work.
  • One year of paid employment/professional experience providing case management services, excluding internships, AND a Bachelor degree in health or human services.
  • Five years of paid employment/professional experience providing case management services, excluding internships.
  • Working knowledge of web-based applications and Google Workspace, Microsoft Office, or similar.
  • Ability to work independently with minimal supervision.
  • Must either reside in Utah or be in the process of relocating to within 30 days of their start date.

Nice To Haves

  • Previous case management experience
  • Working knowledge of HIV
  • Knowledge of or experience with Ryan White Part B
  • Familiarity with social service and support systems within Utah
  • Experience with ClientTrack data management solution

Responsibilities

  • Provide HIV non-medical case management to make sure Ryan White Part B clients enter and remain in HIV medical care and have access to and are adherent on their HIV medications.
  • Provide case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program.
  • Perform client intake.
  • Assist in application and re-certification for the Part B Program.
  • Assess needs utilizing assessments as provided by the Program.
  • Develop care plan.
  • Coordinate care and benefits.
  • Provide and track referrals.
  • Provide internal and external resources.
  • Perform follow-up as needed and indicated by acuity and assessment scores.
