About The Position

This is a Full-Time or Part-Time position for a Non-Medical Case Manager. The role involves providing HIV non-medical case management to ensure Ryan White Part B clients enter and remain in HIV medical care, and have access to and are adherent to their HIV medications. Case management will be performed in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program. The position requires performing client intake, assisting with applications and re-certifications for the Part B Program, assessing needs, developing care plans, coordinating care and benefits, providing and tracking referrals, providing internal and external resources, and performing follow-up as needed based on acuity and assessment scores. The work environment is typically an office setting with adequate lighting, heating, and ventilation, and may involve some walking, standing, bending, carrying light items, and driving an automobile. Special physical demands are not required.

Requirements

• One year of paid employment/professional experience providing case management services, excluding internships, AND a Bachelor of Social Work or a Bachelor degree in health or human services.
• OR Five years of paid employment/professional experience providing case management services, excluding internships.
• Working knowledge of web-based applications and Google Workspace, Microsoft Office, or similar.
• Ability to work independently with minimal supervision.

Nice To Haves

• Previous case management experience
• Working knowledge of HIV
• Knowledge of or experience with Ryan White Part B
• Familiarity with social service and support systems within Utah
• Experience with ClientTrack data management solution

Responsibilities

• Provide HIV non-medical case management to make sure Ryan White Part B clients enter and remain in HIV medical care and have access to and are adherent on their HIV medications.
• Provide case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program
• Perform client intake
